Who Is Eligible

ARMHS eligibility is determined through a Diagnostic Assessment. In general, this service is a fit for people with:

Adults age 18+ in Minnesota
A diagnosed mental illness
Open to setting and working toward recovery goals
Medical Assistance (Medicaid) or eligible insurance

How is the plan created?

It starts with a Diagnostic Assessment and a Functional Assessment, then we build an Individual Treatment Plan together. The plan is reviewed and updated regularly with your input.

Can my family or supports be involved?

Yes — with your consent, we can coordinate with anyone in your support system. Your voice always leads the plan.
